Men’s Uniform Contract
Name:_______________________________________________ is assigned the uniform listed below for the 2009 - 2010 school year. Upon receipt of this uniform I agree to the following:
1. To be responsible for the safe keeping of this uniform.
2. To maintain this uniform in good condition at all times, including having repaired and dry cleaned as deemed necessary or requested by the director.
3. To have the uniform evaluated at the end of the contract period and pay a fair assessment for all damages in excess of normal wear.
4. To allow only the student named above to wear this uniform and for chorus functions only.
5. To pay current value in the event of loss or total damage.
SHIRTS: White wing collar shirts with pleated front. (Purchase through Chorus)
BLACK SHOES and BLACK SOCKS must be worn with the uniform.
All items must be procured prior to the Fall Concert on October 22, 2009.
UNIFORM RETURNS
DATES and TIMES will be announced in March 2010. Late fees may be charged.
Replacement costs will be charged for any uniform damaged or not returned.
Complete Tuxedo - $100.00
Grades and or diplomas will be held by administrators for failure to return the uniform or failure to pay any levied assessments. Any questions should be referred to Kellie Nienajadly.
You are responsible for these uniform items. If they are not returned you must pay the replacement cost. (#5 above)
